The Innovation Health Connected Plan is helping deliver accessible, cost-effective health care.
OCTOBER 19, 2021 — The Innovation Health Connected Plan is an offering that will deliver Innovation Health® and CVS Health® capabilities in the Northern Virginia market to help members access care when and where they need it, and at a more affordable price than a traditional plan.
The plan combines Innovation Health’s cost-saving performance network with expanded services at CVS Health retail, CVS Pharmacy®, and at other in-network locations. With this, members can enjoy peace of mind through quality in-network providers and convenient access to pharmacy and retail services in their neighborhood.
Along with a better-connected member experience, this plan offers mid-sized businesses enhanced savings relative to Innovation Health’s Open Access HMO plans in the market. Quoting is available now for employer groups with 101-300 eligible employees with January 1, 2022 and later effective dates.

- Integrated data and technology for a seamless care experience.
- Nurse Concierge (INC) and Multi-Disciplinary Care Teams (MDCT) that provide local care and navigation support.
- Access to Pharmacist Panels – personalized counseling for high-risk members so pharmacists can introduce them to resources and options for care.
- Member prescriptions can have 90-day refills and they can set their preferences to have it delivered directly to their door at no cost.
